    The Client agrees that by signing this form, they are instructing OTBW to exercise their dog off lead. The Client agrees that they will not hold OTBW liable for any loss or damage caused as a result of their dog being off lead.

    I give my permission and written consent to OTBW to exercise my dog off lead and understand and accept the above statement and that all the terms and conditions remain the same.

    Key Waiver

    I understand that this form acts as permission for OTBW to hold keys/gain access to my property for the purpose of providing services booked for my pets, which I have provided willingly.

    I agree that by providing OTBW with keys/access to my property, I will not hold OTBW liable for any loss or damage to the property or its contents, in present and future circumstances.
